Job summary

La Clinica de Familia is seeking a CCBHC Patient Specialist to deliver comprehensive nursing services, including assessment, planning, implementation, evaluation, and coordination of care under the CCBHC model. You will work with a multidisciplinary team across behavioral health, physical health, and social services, addressing social determinants of health and connecting clients with community resources.

A Bachelor’s degree in a health field and at least one year of clinical care experience are

Qualifications

Bachelor’s degree in health-related field; nursing or social work preferred.
Minimum one year of experience in a clinical care setting.
Community Health Worker Certification or able to obtain within first 3–6 months of employment.
Experience with diverse populations, including behavioral health or substance use needs, preferred.

Responsibilities

Provide professional nursing assessment, planning, implementation, evaluation, and coordination of patient care.
Support individuals navigating behavioral health, physical health, and social services.
Collaborate within a multidisciplinary CCBHC team to improve health outcomes.
Address social determinants of health and connect clients with community resources.
