La Clinica de Familia in Southern New Mexico is seeking a CCBHC Patient Specialist to provide full nursing services, including assessment, planning, implementation, evaluation, and coordination of care under the Clinical Team Lead.

You will work with a multidisciplinary team to support behavioral health, physical health, and social services, address social determinants of health, identify barriers, and connect patients with community resources.

Qualifications

• Bachelor's degree required; Nursing or Social Work preferred.
• Minimum 1 year of clinical care experience.
• Community Health Worker certification or ability to obtain within first 3-6 months.
• Experience with diverse populations, including behavioral health or substance use needs preferred.

Responsibilities

Job Summary

The CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.

Job Requirements
