Job summary

La Clinica de Familia in Southern New Mexico is seeking a CCBHC Patient Specialist to provide full nursing services, including assessment, planning, implementation, evaluation, and coordination of care under the Clinical Team Lead.

You will work with a multidisciplinary team to support behavioral health, physical health, and social services, address social determinants of health, identify barriers, and connect patients with community resources.

Qualifications

• Bachelor's degree required; Nursing or Social Work preferred.
• Minimum 1 year of clinical care experience.
• Community Health Worker certification or ability to obtain within first 3-6 months.
• Experience with diverse populations, including behavioral health or substance use needs preferred.

Responsibilities

• Provide full range of nursing services: assessment, planning, implementation, evaluation, and coordination of patient care.
• Support individuals navigating behavioral and physical health and social services systems.
• Work with a multidisciplinary team to improve health outcomes and promote recovery and wellness.
• Address social determinants of health and connect patients with community resources.
