Job summary

La Clinica de Familia in Southern New Mexico is seeking a Patient Navigator–Community Health Worker to support clients navigating behavioral and physical health and social services. You will focus on engagement, care coordination, advocacy, and ensuring timely, culturally responsive, person-centered services.

In a multidisciplinary CCBHC team, you will work to reduce barriers to care, promote recovery, and identify social determinants of health.

Qualifications

• High school diploma or equivalent required.
• Minimum 3 years of direct patient care experience.
• CHW Certification or ability to obtain within 3–6 months of employment.
• Medical Assistant Certification preferred.
• Experience with diverse populations, including behavioral health or substance use needs preferred.

Responsibilities

Job Summary

Under the direct supervision of the Clinical Team Lead, the Patient Navigator–Community Health Worker plays a key role in supporting clients as they navigate behavioral health, physical health, and social service systems. The Patient Navigator will focus on patient and community engagement, outreach, care coordination, and advocacy. Ensuring individuals receive timely, culturally responsive and person-centered services. The Patient Navigator will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.
