Job summary

Legacy Community Health in Houston, TX seeks a Bilingual Care Team Assistant to connect patients with resources and support them throughout their visit. You will be the frontline contact, coordinating information and guiding families with compassion.

Responsibilities include patient communication, pre-visit planning, care coordination, medication management, and referrals. Fluency in Spanish is required; HS diploma and healthcare experience preferred; schedule Monday–Friday, 8:30 AM–5:00 PM,

Qualifications

• High-school level education or equivalency.
• Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting.
• Must maintain BLS / CPR.
• At least 2 years of experience in an ambulatory clinical setting preferred.
• Spanish fluency required.
• Demonstrated competency with an EHRS system.
• Excellent communication skills.
• Excellent customer service skills.
• Ability to multi task in a fast paced environment.
• Strong typing and computer skills.

Responsibilities

• Patient communication with onsite nursing or protocol-based triage.
• Pre-visit planning: review schedules, ensure labs and reports are available, assist with interpretation services.
• Care coordination and transitions of care.
• Medication management: refills and prior authorizations.
• Form handling: initiate and finalize forms with provider signature.
• Referrals: complete referrals and deliver clinical summaries.
• Safety and compliance: adhere to safety training and equipment operation.
