Job summary

Legacy Community Health seeks a Health Navigator (Float) responsible for traveling between Legacy clinics to support patient education, HIV prevention, and seamless access to care. The role emphasizes community engagement, one-on-one and group education, and collaboration with the care team.

The position offers Monday–Friday hours, with a focus on patient-centered care and proactive linkage to services across locations in the Houston area.

Qualifications

• Excellent communication and customer service skills.
• Experience with HIV prevention and health education preferred.
• Ability to work across clinical and community settings.

Responsibilities

• Develop and implement health education programming for individuals and groups.
• Use motivational interviewing to support health goals.
• Provide information, referrals, and assistance to improve access to services.
