The role blends clinical coordination, patient communication, and clerical tasks to ensure timely lab results, medication management, referrals, and pre-visit planning. Join a mission-driven team improving community health.

Qualifications

• Spanish fluency required; service location dependent.
• Medical Assistant Certification or 2+ years clinical experience.
• Proficiency with EHRS and strong communication skills.

Responsibilities

• Handle patient calls and triage with the care team.
• Document provider notes in EHRS and route inquiries.
• Follow up with patients after visits and labs.
• Assist with lab results, referrals, and medication management.
• Coordinate pre-visit planning and ensure timely information.
• Support care transitions and discharge communications.
• Maintain a safe, patient-centered clinic environment.
