Job summary

Legacy Community Health is seeking a Bilingual Pediatrics Care Team Assistant (CTA) to serve as a trusted contact for patients, families, and the care team at our Houston clinic. You will support calls, labs, medication management, referrals, and pre-visit planning to help deliver high-quality, patient-centered care in alignment with PCMH guidelines.

You will combine clinical coordination with clerical tasks, coordinate with providers and front-desk staff, assist with documentation, and help

Qualifications

• High-school level education or equivalent.
• Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting.
• Must maintain BLS / CPR

Responsibilities

• Patient communication and triage per protocol.
• Route clinical inquiries to providers via EHRS notes.
• Return patient calls as directed by provider.
• Prepare and deliver lab results/communication to patients.
• Coordinate transitions of care and referrals.
• Pre-visit planning and schedule review.
• Ensure lab/imaging/results are documented in patient records.
• Assist with forms and authorizations as needed.
• Support medication refills and prior authorizations.
• Maintain Care Coordination Desktop and related records.
