Job summary

Legacy Community Health in Houston is seeking a Care Team Assistant – Bilingual Spanish to support patients, families, and caregivers, the call center, and the care team. You will help navigate care and ensure PCMH requirements.

You will coordinate clinical tasks, communications, and clerical duties, including lab results, medications, referrals, and pre-visit planning, while maintaining compassionate, patient-centered service.

Qualifications

• High-school level education or equivalency.
• Medical Assistant Certification or equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting.
• Maintains BLS/CPR certification.

Responsibilities

• Patient communication and protocol-based triage with onsite nursing or call center guidance.
• Route clinical inquiries to the provider and document in EHRS.
• Return patient calls as directed by the provider.
• Place calls or letters regarding results and deliver to patients promptly.
• Contact medically vulnerable patients after visits for ongoing monitoring as directed.
• Pre-Visit Planning: review schedule to promote completed pre-visit work.
