Job summary

Legacy Community Health in Houston seeks a Bilingual Care Team Assistant to connect patients with resources and support them throughout care. You will be the main contact for patients, ensuring compassionate, high-quality service and accurate information, with a focus on language support and care coordination.

You will collaborate with onsite nurses and providers, manage scheduling, and facilitate referrals and documentation to support seamless patient care in a fast-paced environment.

Qualifications

• High-school level education or equivalency.
• Medical Assistant Certification.
• Must maintain BLS/CPR.
• At least 2 years of experience in an ambulatory clinical setting preferred
• Spanish fluency.
• Demonstrated competency with an EHRS system.
• Excellent communication skills.
• Excellent customer service skills.
• Ability to multi task in a fast paced environment.
• Above average organizational skills.
• Discretion with confidential information.
• Must be able to manage multiple deadlines and decisions.

Responsibilities

• Patient communication: work with onsite nursing or follow protocol-based triage to address patient phone calls and route clinical inquiries to providers.
• Pre-visit planning: review patient schedules daily, ensure lab results and consultation reports are available, and gather necessary information.
• Care coordination: manage transitions of care, support access to ancillary services, and maintain the Care Coordination Desktop.
• Medication management: assist with medication refills and prior-authorizations.
• Forms: initiate and complete forms for review and final signature by the provider.
• Referrals: complete referrals, deliver clinical summaries for consultants, and ensure relevant documents are included in patient records.
• Safety and compliance: promote effective working relations to facilitate goal achievement and adhere to safety training and equipment operation.
