Job summary

AccentCare is seeking an Intake/Patient Care Liaison for the Brooklyn Office to manage referrals, intake, and care coordination. This full-time, on-site role requires bilingual English/Spanish and strong customer service skills.

You will verify eligibility, schedule caregivers, support EMR documentation, and ensure a positive care experience for clients and families in a dynamic home health setting.

Qualifications

• High School diploma or GED is required.
• 1-3 years of experience in patient care or intake.
• Bi-Lingual English/Spanish is required.
• Experience in home health care is desirable.

Responsibilities

• Serve as primary contact for new client referrals and service requests.
• Monitor intake systems and promptly assign cases to care teams.
• Verify insurance eligibility and authorizations to support care.
• Process referrals and authorizations within deadlines.
• Coordinate onboarding calls and caregiver scheduling.
• Maintain EMR documentation and client communications.
• Support coordination with billing, EVV, and payer relations.
• Conduct client satisfaction follow-ups to improve care.
