Job summary

New York Community Care in Brooklyn is seeking a full-time Intake Coordinator who speaks Spanish to join our team. The role focuses on handling referrals, verifying insurance, and educating patients about available services for home care.

The ideal candidate will have knowledge of HHA Exchange and at least 1 year of intake experience, with a commitment to helping clients access both skilled and non-skilled services. This position is in-person at our Brooklyn office.

Qualifications

• Fluency in Spanish is required.
• Experience in intake processes.
• Knowledge of HHA Exchange software.

Responsibilities

• Receive referrals from referral sources.
• Process referrals from marketing sources (hospitals, nursing homes, physicians).
• Verify patient insurance to determine eligibility.
• Educate patients on services we provide.
• Determine what services were approved by insurance.
• Contact patients to educate them on services and assist with Home Care needs for skilled and non-skilled services.
