Job summary

New York Community Care in Brooklyn is seeking a full-time Intake Coordinator who speaks Spanish to join our Brooklyn office. The Intake Coordinator plays an integral role in the Intake Department, and our agency has won awards for employee satisfaction and excellence in patient care.

The role supports processing referrals, verifies insurance eligibility, educates patients on services, and helps determine what services were approved by insurance for Home Care needs.

Qualifications

• Must speak Spanish.
• Knowledge of HHA Exchange.
• 1 year of Intake Experience (Preferred).

Responsibilities

• Receive referrals from referral sources.
• Process referrals from marketing sources such as hospitals, nursing homes, and community physicians.
• Verify patient’s insurance to determine eligibility
• Educate patients on services we provide
• Determine what services were approved by insurance
• Contact patients to educate them on the services we can provide and assist them with their Home Care needs for skilled and non-skilled services.
