Responsibilities include verifying insurance eligibility, educating patients about services, and coordinating with care teams to determine approved services.

Qualifications

• Must have knowledge of HHA Exchange.
• 1 year of Intake Experience.
• Spanish speaking is required.

Responsibilities

• Receive referrals from referral sources.
• Process referrals from hospitals, nursing homes, and community physicians.
• Verify patient’s insurance to determine eligibility.
• Educate patients on services we provide.
• Determine what services were approved by insurance.
• Contact patients to educate them on the services we can provide and assist them with their Home Care needs for skilled and non-skilled services.
