Qualifications

• High school diploma or GED required.
• Fully bilingual in Spanish.
• 2+ years in a patient-facing or customer-facing role with activity tracking.
• Ability to work independently on-site with limited supervision.
• Experience with a CRM or case management system; comfortable with iPad/iPhone.

Responsibilities

• Engage patients and families from hospital referrals; explain HIPP clearly and compassionately.
• Own your caseload from first contact to enrollment; gather documentation for applications.
• Coordinate with hospital staff and PFA teams to move applications forward.
• Maintain patient referrals and records in PFA's CRM; ensure HIPAA-compliant documentation.
• Represent PFA on-site as the primary hospital contact; build relationships with care teams.
