Job summary

CalOptima is seeking a Social Worker (PACE) who is bilingual to join the PACE Clinic team in Orange, CA. The role focuses on care management, counseling, and referrals for elderly participants within the PACE program.

The successful candidate will hold an MSW, have 2 years in health care, and at least 1 year working with the elderly. A valid CA driver’s license and CPR/First-Aid are required, with some travel and potential telework within California.

Qualifications

• Master's degree in social work from an accredited program.
• 2 years of experience in a health-related field.
• 1 year of experience working with the elderly population.

Responsibilities

• Provide direct social work care management services including screening, case management and counseling.
• Refer participants to PACE and community resources, advocating for participant needs.
• Collaborate with the IDT and participate in assessments and care planning.
• Support enrollment, discharge planning, and home visits as needed.
• Coordinate transportation and communication with families and partners.
