Job summary

Qualifications

• Master’s degree in social work from an accredited school of social work.
• One year experience working with a frail and/or elderly population preferred.
• Demonstrated ability to work in a multicultural and multidisciplinary team environment.
• Ability to work with minimal supervision.
• Excellent oral and verbal communication and meeting facilitation skills.
• Experience working with severe mental illness is desirable.
• Bilingual ability preferred (Spanish, Korean, Cantonese, and/or Mandarin).
• Valid California driver's license, acceptable driving record and insurance with access to reliable transportation.
• Current CPR and First Aid

Responsibilities

• Develops and implements psychosocial assessments and plans of care.
• Executes and updates care plans, and documents progress.
• Interdisciplinary Team (IDT) communication with participants, families, and CEI staff.
• Conducts 30/60/annual reviews and conferences with families.
• M programs for referrals to community resources (financial management, benefits).
• Maintains confidentiality per HIPAA/CEI policies.
• Performs home visits and travels to meet participants and providers.
• Responds to elder abuse complaints and documents concerns.
• Presents cases to IDT and coordinates discharge planning with hospitals/SNFs.
