Job summary

CalOptima Health invites applications for a Social Worker (PACE) role, bilingual required, to join the PACE Clinic team and provide direct social work care management services. The position supports participant screening, case management, counseling and referrals, with travel in Orange County as needed.

Responsibilities include IDT participation, care planning, safety assessments, and coordination with community partners.

Qualifications

• Master’s degree in social work from an accredited program and at least 2 years in a health-related field.
• Bilingual in English and one threshold language (Arabic, Farsi, Chinese, Korean, Russian, Spanish, Vietnamese).
• CPR and First-Aid certification required or obtained within 6 months of hire.
• Valid driver’s license and vehicle for travel 30%+ of the time.

Responsibilities

• Provide social work care management services including screening, case management, counseling and referrals within the PACE program.
• Participate in IDT assessments, care planning, and documentation in medical records.
• Assist in discharge planning, care coordination, and advocacy for participants and families.
• Collaborate with community agencies, schedule home visits, and support enrollment and enrollment processes.
