Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time and assisting with CPT and ICD-10 coding. The role emphasizes accuracy in notes and collaboration with clinicians within our clinics.

Ideal candidates have medical terminology knowledge, prior clinical exposure, and strong typing skills. This full-time position offers in-person work and a comprehensive benefits package pursuant to company policy.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering, strongly preferred.
• Prior scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in a structured note including HPI, assessment, plan, and exam.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries.
• Observe patient visits in the exam room and document accordingly.
• Consult with provider to ensure accurate and specific documentation.
• Review medical records and support documentation improvement.
• Support Daily Huddles and Clinical Documentation Reviews.
• Place orders and referrals; address routine tasks.
• Support the care team with documentation-related responsibilities.
