Job summary

Oak Street Health is seeking a Medical Scribe to support our primary care providers with real-time documentation, enabling clinicians to focus on delivering high-quality patient care. Scribes observe visits, document encounters in structured notes, and assist with post-visit steps.

Beyond basic scribing, you will become proficient in ICD-10/CPT coding and value-based care documentation, supporting the care team and provider with timely information.

Qualifications

• Advanced listening and communication skills are required.
• Strong computer literacy and ability to learn new workflows are required.
• Fluency in Spanish, Polish, Russian, or other languages spoken by communities we serve is required where indicated.

Responsibilities

• Documenting patient encounters in real time in structured notes (history, assessment, plan, exam).
• Assigning appropriate CPT and ICD-10 codes and preparing after-visit summaries.
• Supporting daily huddles, clinical documentation reviews, and opportunities for improvement with providers.
• Administrative tasks supporting the care team, including referrals and other duties as assigned.
