Job summary

Oak Street Health is seeking a Medical Scribe (Bilingual Preferred: English/Spanish) to support clinicians with real-time documentation and coding. The role involves shadowing providers in the exam room, producing structured notes, and ensuring accurate CPT/ICD-10 coding.

The position is full-time, Monday–Friday, 8am–5pm, with a comprehensive benefits package and opportunities to learn value-based care workflows.

Qualifications

• Advanced listening and communication skills.
• Strong computer literacy and ability to learn new technical workflows.
• Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve.

Responsibilities

• Joining the provider in the exam room to observe patient visits
• Documenting encounters in a structured note including HPI, assessment, plan, and exam
• Assigning CPT and ICD-10 codes
• Preparing After Visit Summaries
• Consulting with provider to ensure accurate documentation
• Requesting and reviewing medical records
• Leveraging population health tools to support documentation improvement
• Preparing for and supporting Daily Huddles and Clinical Documentation Reviews
• Consulting with provider on documentation opportunities
• Placing orders and referrals
• Addressing tasks
• Supporting the care team with additional responsibilities related to clinical documentation
• Other duties as assigned
