Job summary

Oak Street Health in Berwyn, IL is seeking a full-time Medical Scribe (Spanish bilingual) to support primary care providers by documenting patient encounters in real time and preparing after-visit summaries. The role includes observing visits in the exam room and ensuring accurate, structured documentation with CPT/ICD-10 codes.

Ideal candidates have Spanish fluency, strong listening and communication skills, and knowledge of medical terminology.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience preferred.
• Scribe or transcription experience preferred.

Responsibilities

• Document patient encounters in real time.
• Join provider in exam room to observe visits.
• Document encounters with HPI, assessment, plan, and exam.
• Assign CPT and ICD-10 codes.
• Prepare after-visit summaries.
• Support clinical documentation improvement with data tools.
• Coordinate with provider on documentation opportunities.
• Provide administrative support to the care team.
