Job summary

Oak Street Health in the United States is seeking a Medical Scribe to support primary care providers with real-time clinical documentation. The role involves observing visits, documenting encounters, coding with CPT and ICD-10, and preparing after-visit summaries.

This full-time in-clinic position requires 40–45 hours per week and a commitment of at least 1 year. Spanish fluency is required where indicated, with other language skills welcome, and CVS Health benefits apply.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing or volunteering, preferred.
• Prior scribe or transcription experience is a plus.
• US work authorization is required.

Responsibilities

• Document patient encounters in structured notes with history, assessment and plan.
• Assign CPT and ICD-10 codes accurately.
• Prepare After Visit Summaries and support clinical workflows.
• Assist with daily huddles and documentation reviews.
• Provide administrative support to the care team.
