Job summary

Oak Street Health in Bronx, NY is hiring a Medical Scribe (Bilingual Preferred English & Spanish) to support primary care providers with clinical documentation. You will join visits, document encounters with structured notes, and assist with CPT/ICD-10 coding while learning value-based care workflows.

The role is full-time, 8am–5pm, Monday through Friday, with extensive on-the-job training and opportunities in clinical documentation improvement and population health data.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering.
• Prior scribe or transcription experience is preferred but not required.

Responsibilities

• Joining the provider in the exam room to observe patient visits.
• Documenting patient encounters in a structured note, including history, assessment, plan, and exam.
• Assigning CPT and ICD-10 codes.
• Preparing After Visit Summaries.
• Consulting with provider to ensure accurate and specific documentation.
• Clinical Documentation Improvement ~ 10% — reviewing records and leveraging population health tools.
• Administrative support for the provider and care team ~ 10% — placing orders and referrals, addressing tasks.
