Job summary

Oak Street Health is seeking a Medical Scribe (Spanish bilingual) to support our primary care providers by documenting patient encounters in real time and aiding in comprehensive clinical documentation. You will observe exams, prepare after-visit summaries, and ensure accurate CPT/ICD coding within a value-based care framework.

The role requires strong listening and communication skills, proficiency with EMR systems, and the ability to work full-time in our Chicago clinics.

Qualifications

• Knowledge of medical terminology and common medications.
• Clinical experience through shadowing or volunteering preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in real time with structured notes including history, assessment, plan and exam.
• Assign appropriate CPT and ICD-10 codes and prepare after-visit summaries.
• Support clinical documentation improvement and review medical records as needed.
• Provide in-person administrative support to the care team and assist with referrals and tasks.
