Qualifications

• Requires BA/BS degree or equivalent background in health/social field.
• Minimum 2 years of experience with a social work agency or equivalent.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Conduct face-to-face program assessments for identification and planning.
• Coordinate member cases with clinical teams to support integrated care.
• Manage non-clinical needs to ensure efficient utilization of LTSS.
• Document member goals and update care plans with the care team.
• Submit necessary utilization and authorization requests with supportive documentation.
• Assist with appeals, grievances, and state audits as needed.
