Job summary

RiseBoro Community Partnership seeks an Intake Specialist to act as the primary contact for the Atlas, SEPS, SSVF, Homebase and STEHP programs. You will screen for eligibility, schedule intakes, and coordinate referrals with staff, while handling data entry and client communications.

Your role includes outreach to promote services, maintaining a welcoming reception area, and ensuring documentation compliance per contract guidelines. Proficiency in English and Spanish is required.

Responsibilities

• The Intake Specialist is the primary contact for multiple programs and coordinates the intake process.
• Reviews eligibility documentation and schedules appointments for services.
• Performs data entry, filing, copying and faxing of documents and maintains logs.
• Screens individuals for eligibility and determines next steps based on evaluation.
• Performs outreach to publicize services and ensures reception area is organized.
• Coordinates with supervisor to schedule new client intakes and referrals.
