Job summary

RiseBoro seeks a full-time on-site Case Manager to coordinate day-to-day tenant services for a caseload of 20-25 households. You will ensure services reflect RiseBoro's mission and maintain detailed resident records.

You will conduct needs assessments, develop tenant-centered plans, provide case management and advocacy, and assist with government benefits to keep residents housed and supported. Bilingual Spanish/English required; degree in social work, psychology, or related field preferred.

• Bilingual in Spanish and English required.

Responsibilities

• Coordinate on-site services for tenants on a caseload of 20-25 households.
• Complete needs assessments and develop tenant-centered support plans.
• Provide ongoing case management, advocacy and referrals to community services.
• Ensure tenants maintain housing subsidy and benefits to remain housed.
• Document services and maintain resident records and eligibility information.
• Collaborate with partners to help residents age in place and access resources.
