Qualifications

• Bachelor’s Degree required; degree in social work, psychology, or related field preferred.
• Bilingual in Spanish and English required.
• Six months or more of human service experience preferred.
• Experience with homeless population and/or seniors and related health/wellness needs.

Responsibilities

• Coordinate day-to-day on-site services for caseload of approximately 20–25 households.
• Complete needs assessments and develop tenant-centered support plans; track progress.
• Provide ongoing case management, advocacy, and information/referrals to community services.
• Assist tenants in obtaining and maintaining government benefits and housing subsidies.
• Educate residents on available on-site services and community resources.
• Maintain up-to-date resident records, eligibility data and service documentation.
• Collaborate with property management and community partners to promote aging in place.
• Participate in case conferences and coordinate with program staff for outcomes.
• Flexible hours including some evenings/weekends as needed.
