Responsibilities

• Respond to referrals and intake requests from various sources.
• Verify demographic, insurance, physician, and delivery information.
• Coordinate admission and onboarding activities for services.
• Communicate with patients and caregivers to assess ongoing needs.
• Obtain and process physician documentation and authorizations.
• Coordinate with insurance providers regarding eligibility and authorization requirements.
• Document and route communications per policies.
• Conduct patient follow-up and referral status communications.
• Collaborate with internal teams, referral sources, and patients to resolve issues.
• Maintain compliance with company policies and quality standards.
• Perform additional duties as assigned.
