Job summary

Maxim Healthcare is seeking a dedicated team member in California to support patient referrals, service requests, and coordination with operations. You will help open new cases in patient homes and ensure timely communication with management and staff.

Qualifications

• High School Diploma or equivalent required; LPN licensure and/or college degree preferred
• Minimum of 1 year of relevant experience preferred
• Ability to travel up to 25%
• Strong organizational and time management skills
• Excellent communication and customer service abilities
• Professional telephone manner and interpersonal skills
• Proficiency in Microsoft Office Suite (Word, Excel, Teams)
• Ability to lift up to 15 pounds
• Strong written and verbal communication skills in English and Spanish REQUIRED

Responsibilities

• Capture and communicate patient referral information to management teams
• Process and document incoming patient needs, service requests, and changes
• Inform patients accurately about available services
• Collaborate with operations to schedule appropriate personnel promptly
• Assist with new case openings in patient homes
• Record and report patient and employee concerns to appropriate partiesEnter patient data into the system of record per company policies
• Support office staff with routine administrative duties
• Perform other duties as assigned
