Cornerstones, Inc. is seeking a Housing Focused Case Manager to coordinate services for adults at the Embry Rucker Community Shelter in Reston, VA. Qualifications

• Bachelor’s degree in an applicable human services field or commensurate homeless service experience.
• Bilingual English/Spanish preferred; English/Arabic or English/French also mentioned.
• Experience with Housing First and Rapid Re-Housing philosophy.
• Knowledge of homelessness strategies and trauma-informed care essential.
• Knowledge of tenant rights and case management practices.

Responsibilities

• Manages an average caseload of 12-15 adults experiencing homelessness at ERCS.
• Conducts comprehensive assessments using standard intake tools.
• Develops and monitors individualized housing stability plans.
• Educates participants about affordable housing options and assists with housing search.
• Advocates for services and coordinates with partner organizations.
• Maintains HMIS records and documents outcomes.
