Job summary

Cornerstones in Reston, VA seeks a Case Manager for Rapid Rehousing to connect individuals and families with housing solutions using Housing First. You will assess needs, develop plans, coordinate services, and support clients from homelessness toward permanent housing, with trauma-informed care and equity at the core.

The role manages a caseload of 20–25 households, works with shelters and housing locators, uses HMIS for data, and may require evenings or weekends.

Qualifications

• Bachelor’s degree in a human services field or commensurate experience.
• Master's degree preferred for advancement.
• MSW and two years post-master’s experience are a plus.

Responsibilities

• Manages an average caseload of 20–25 households in Rapid Rehousing.
• Conducts comprehensive assessments and develops individualized housing plans.
• Coordinates with shelters, housing locators, and community partners to secure housing and services.
• Maintains HMIS records, tracks outcomes, and ensures contractual obligations are met.
• Provides crisis intervention and trauma-informed support as needed.
