Job summary

Saint Francis House Inc. seeks a Bilingual Housing Stabilization Case Manager to support residents transitioning from homelessness to permanent housing.

Working from a Housing First perspective, you will help residents build skills and access resources for long-term housing stability, income, and behavioral health. Responsibilities include caseload management for about 15 residents, outreach, assessments, service plans, daily living skills coaching, and collaboration with landlords and partners.

Qualifications

• Bachelor’s degree in social work or related field; 3+ years preferred.
• Direct service or case management with vulnerable populations preferred.
• Lived experience with homelessness or recovery a plus.

Responsibilities

• Welcome new residents and support transition to stable housing.
• Manage a caseload of approximately 15 residents with ~30% high-level need.
• Build proactive relationships and connect residents to services.
• Develop service plans focusing on housing, income and behavioral health stability.
• Support development of daily living skills and tenancy.
