Job summary

The People Concern in Los Angeles seeks an ICMS Bilingual Case Manager to help homeless Clients achieve housing stability using evidence-based practices. This onsite role partners with DMH, DHS, and community providers to support housing placement and retention, while promoting independent living skills and community integration.

Qualifications

• High School Diploma or GED or equivalent.
• Ability to work in a harm reduction model with clients facing multiple barriers.
• Valid CA driver's license, reliable car, auto insurance, and acceptable driving record.

Responsibilities

• Maintain a caseload of high-acuity clients for intensive case management.
• Develop relationships to promote independence and health.
• Coordinate with DMH and DHS programs for treatment team integration.
• Engage clients with harm-reduction and trauma-informed care approaches.
• Assist in screening, intake, and orientation to program policies and goals.
• Complete biopsychosocial assessments to guide treatment planning.
• Develop quarterly collaborative goals to improve quality of life and health outcomes.
• Document progress notes and ensure chart completeness and confidentiality.
• Coordinate appointments and transportation to access primary and mental health services.
• Transport clients in personal vehicle within the LA area to improve access to housing.
