Legacy Community Health seeks a Health Navigator - Float to travel between Legacy clinics, support patients, and connect them with essential health services, including HIV prevention and treatment resources. In this role you will develop health education programs, use motivational interviewing, and work with a multidisciplinary team to improve patient outcomes and access to care.

Qualifications

• High School Diploma or equivalent.
• 3–5 years of relevant experience.
• Bachelor's Degree in a related field preferred.
• Experience with point-of-care testing in clinical/community settings.
• Demonstrated HIV care and prevention experience.
• Experience educating diverse clients in one-on-one or group settings.
• Proficient with Electronic Health Records Systems (EHRS).
• Strong communication and customer service skills.
• Bilingual English/Spanish preferred.

Responsibilities

• Develop and implement health education programming in one-on-one and group settings.
• Utilize motivational interviewing to support patients in health goals.
• Provide information, referrals, and assistance to improve access to services.
• Conduct point-of-care testing for HIV and other health conditions.
• Participate as a member of the interdisciplinary clinic team.
• Assess patient needs across behavioral, social, and medical factors.
• Establish referrals and resource lists to link patients to information.
• Promote patient-centered care within a primary care medical home.
• Document services in EMRs and manage patient schedules.
• Meet performance metrics related to grants and contracts.
• Travel between Legacy locations to meet community needs.
