Qualifications

Responsibilities

• Schedule and coordinate medical, behavioral health, and social service appointments for clients.
• Provide safe and timely transportation to and from appointments and events.
• Assist clients in understanding care plans and navigating healthcare systems.
• Represent the organization at community events, health fairs, and outreach activities.
• Educate clients on preventive care, chronic disease management, and mental health awareness.
• Maintain accurate records of client interactions, appointments, and services.
