Living Water Clinic – Clínica Agua Viva is seeking a Community Health Worker (CHW) / Promotora de Salud to connect patients with healthcare services, education, resources, and programs. The role emphasizes bilingual engagement in English and Spanish and working outside clinic walls at events, schools, churches, and community sites.

Qualifications

• High School Diploma or equivalent preferred.
• CHW/Promotora certification or training preferred.
• Willingness to complete company-sponsored CHW/Promotora training.

Responsibilities

• Represent Living Water Clinic professionally and positively in the community.
• Participate in and help coordinate community health fairs and outreach events.
• Educate community members about healthcare services and programs in English and Spanish.
• Assist patients with understanding and accessing healthcare resources.
• Help patients complete forms and use patient portals and clinic applications.
• Promote preventive healthcare services and connect individuals with appropriate programs.
