Job summary

Pair Team is seeking a Bilingual Field Care Coordinator in California to bring hands-on, field-based support to high-need members. You will conduct in‑person visits, onboard new patients, and deliver care packages while navigating social and health barriers.

The role requires regular travel within Orange County and a schedule of M-F, 9am-5:30pm, with gas/mileage reimbursements. Fluency in Spanish is required.

Qualifications

• 1+ years of field (in person) case management or care coordination.
• Must reside in or near Orange County and be familiar with local resources.
• Reliable transportation and valid driver's license are required.
• Strong cultural fluency and empathy for diverse populations.
• Ability to collaborate within a multidisciplinary care team.
• Excellent organizational skills and attention to detail.
• Comfort with Google Suite, Slack, CRM, Excel, and video calls.
• HIPAA-compliant remote workspace and reliable internet.

Responsibilities

• Serve as the primary contact for members with medical, behavioral, and social needs.
• Meet members in homes, community sites, or partner organizations for onboarding and care delivery.
• Conduct in‑person visits, assess needs, and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across care domains.
• Collaborate with Nurses and Behavioral Health Managers to maintain care continuity.
• Identify barriers to care and address transportation or literacy challenges.
• Develop individualized care plans with follow‑ups on interventions.
