Upward Health, Inc. is seeking a Community Outreach Specialist to educate communities about our in-home care programs and enroll patients in our services.

The COS will manage a personal caseload, primarily via phone outreach, with in-person visits as needed, and will work under the Outreach Manager to ensure successful patient engagement. The role requires strong communication, relationship-building, and organizational skills, along with proficiency in English and Spanish.

Qualifications

• Excellent verbal communication and relationship-building abilities.
• Ability to manage a personal caseload with strong organizational skills.
• Self-motivated and able to meet outreach goals in a fast-paced environment.
• Fluency in English and Spanish is required or strongly preferred.
• Experience using computer systems for documentation and outreach activities.

Responsabilités

• Educate potential patients about Upward Health’s services and programs.
• Engage individuals in meaningful conversations and facilitate enrollment.
• Maintain a personal caseload with outreach through phone, in-person visits, and mailings.
