Job summary

Upward Health, Inc. seeks a Community Outreach Specialist to establish the company’s presence in the community, educate potential patients, and guide enrollment into programs. The COS will manage a personal caseload, primarily via phone outreach, with in-person visits and other community strategies as needed.

The role reports to the Outreach Manager and requires strong communication, organizational, and bilingual (English/Spanish) abilities to engage diverse patients and drive enrollment.

• Fluent in English and Spanish.

Responsibilities

• Manage a personal caseload and conduct outreach via phone and in-person visits.
• Educate potential patients about Upward Health’s services and enrollment steps.
• Coordinate enrollment into programs and follow up with patients.
• Collaborate with Outreach Manager and team to meet outreach goals.
• Maintain accurate documentation in the company systems.
