Job summary

Upward Health is seeking a Community Outreach Specialist to establish our presence in the community and educate individuals about our in-home, whole-person care. You will manage a personal caseload using phone outreach and in-person visits to enroll patients into our programs.

The COS role emphasizes building trust, meeting outreach goals, and collaborating with the Outreach Manager and care team to ensure effective patient engagement and service access.

Qualifications

• Excellent verbal communication and persuasive abilities.
• Strong interpersonal skills to build trust quickly.
• Organizational skills to manage a personal caseload.
• Self-motivated and able to work independently.
• Adaptable to phone and in-person outreach methods.
• Fluent in English; Spanish is a plus.
