Job summary

Wider Circle in Lancaster, PA is seeking a Bilingual Community Engagement Specialist to lead Connect For Life, connect neighbors, and promote health equity. The role involves planning events, home visits, and outreach to engage members both in-person and virtually.

You will build relationships, identify leaders, and coordinate partnerships with health plans while tracking engagement metrics. Fluency in English/Spanish is required and 3+ years in outreach are preferred.

Qualifications

• Pennsylvania Community Health Worker Certificate strongly preferred.
• 3+ years in community outreach, teaching, or facilitation.
• Healthcare experience is a plus.
• Associate’s degree preferred; 5+ years of relevant experience may be considered instead of a degree.

Responsibilities

• Lead the Connect For Life program, including events (virtual and in-person) and building a community of connected members.
• Plan and host virtual and in-person events, including venue search, prep, and outreach to attendees.
• Proactively contact members, conduct home visits, and facilitate monthly engagement.
• Record member interactions in Wider Circle systems for tracking and reporting.
• Identify natural leaders to serve as Wider Circle Ambassadors to support community growth.
• Develop personalized outreach plans to enroll new members in the program.
• Serve as liaison to health plans and assist members with care gaps.
