Job summary

National Youth Advocate Program is seeking a Case Manager to complete assessments, develop service plans, and maintain compliant files. The role coordinates with sponsors, families, and the multidisciplinary team to support reunification and post‑care services.

Responsibilities include on-site intake, collaboration with attorneys, and documentation of progress notes, with a focus on safeguarding youth and providing timely releases when appropriate.

Qualifications

• Bachelor’s degree required in Social Work, Psychology, Human Services, Counseling or other social service field.
• One year experience preferred working with children/adolescents or youth services.
• Excellent case management, verbal and written communication skills; able to multitask.

Responsibilities

• Complete timely assessments and service plans.
• Maintain required case files compliant with policy and CQI team.
• Coordinate with sponsors, families, and attorneys for reunification and services.
• Attend staffings and multidisciplinary team meetings.
• Document actions in progress notes and ensure timely releases when required.
