Job summary

National Youth Advocate Program (NYAP) is seeking a Case Manager I for the La Jornada program in Illinois. The role involves completing timely assessments and service plans for youth, maintaining required files in UC Portal, and ensuring compliance with ORR policies.

Flexibility for extended hours, weekends, and on-call duties is required. The candidate will work with a multidisciplinary team, coordinate with sponsors and legal partners, and support reunification and post-release services.

Qualifications

• Must be bilingual in English and Spanish; fluency in Spanish required.
• Excellent case management, verbal and written communication skills.
• Ability to work under stress and manage multiple priorities.

Responsibilities

• Perform case management assessments within ORR timeframes.
• Conduct on-site admission and initial intake interviews of youth and gather sponsorship information.
• Interview sponsors/family members to verify sponsor’s ability to care for minors.
• Coordinate with UC field specialists and lead pre-staffing processes with a multidisciplinary team.
• Attend staffing with CASE coordinators and other stakeholders as needed.
• Coordinate with local pro bono attorneys for “Know Your Rights” presentations and screenings.
• Identify best case management practices with a multi-disciplinary team and maintain collaborative environment.
• Ensure safety and timely release from ORR care; verify birth certificates, income verifications, and background checks.
• Document actions and contacts in progress notes; maintain UC electronic/physical files.
• Submit reunification packets for timely release, including home studies and post-release services.
• Provide weekly updates to youth and family members/sponsors, including two weekly telephonic contacts.
• Travel as needed within the U.S. for reunification or program duties.
• Participate in discharge planning and oversee safety/incident reporting per policies.
• Maintain equipment and documentation; adhere to ORR and NYAP policies.
