Job summary

The Case Manager for La Jornada at NYAP is responsible for completing timely assessments and service plans, uploading documents to ORR’s UC Portal, and maintaining compliant case records. The role requires flexibility with hours, weekends, and on-call duties.

You will interview youth and sponsors, coordinate with multidisciplinary teams, and support reunification efforts, while ensuring safety and adherence to policy.

Qualifications

• Bachelor's degree in Social Work or related field; 1 year youth services preferred.
• Excellent case management and strong verbal and written communication skills.
• Fluent in English and Spanish; CPI certification and First Aid/CPR required.
• Willingness to work flexible hours, including weekends and evenings.

Responsibilities

• Perform work in line with NYAP's mission, values, and philosophies.
• Ensure case assessments are completed within ORR timeframes.
• Conduct on-site intake interviews of youth and gather sponsor information.
• Interview sponsors/family members to assess sponsor suitability.
• Serve as UC case POC for Field Specialists and coordinators.
• Prepare cases for staffing with multidisciplinary team.
• Attend staffings with Case Coordinators and ORR stakeholders.
• Coordinate with local pro bono attorneys for Know Your Rights presentations.
• Collaborate with program staff to implement best practices and maintain a multidisciplinary environment.
• Ensure safe environment and timely release from ORR care per policy.
