Job summary

National Youth Advocate Program has been serving communities since 1978. The Case Manager I for La Jornada is responsible for completing timely assessments, submitting service plans, and maintaining case files in accordance with ORR policies and NYAP standards.

The role requires coordinating with sponsors, families, and multidisciplinary teams, ensuring safety and timely reunification, and maintaining comprehensive documentation across systems.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling or other social service field.
• Excellent case management, verbal and written communication skills.
• Flexible schedule including weekends/evenings.
• Bilingual English/Spanish; fluency required.
• Cleared Level II background check; CPI and First Aid readiness.

Responsibilities

• Conduct on-site admission and initial intake interviews with youth and collect sponsorship information.
• Coordinate with sponsors/family members to assess care, support, and eligibility.
• Prepare cases for staffing with a multidisciplinary team.
• Attend staffing with Case Coordinators and other stakeholders.
• Coordinate with local pro bono attorneys for Know Your Rights presentations and screenings.
• Document actions and contacts in progress notes and UC Portal.
• Submit reunification packets and release requests for review.
• Provide weekly updates to youth and family members.
• Facilitate attorney-client contact as requested by youth.
• Coordinate treatment team meetings with all departments.
• Oversee discharge planning and post-release services.
