Job summary

Devoted Health is seeking a Care Coordinator to support TOC Clinical Operations by coordinating post-discharge planning and patient follow-up. You will assist with inpatient outreach, scheduling post-discharge contacts, and obtaining necessary clinical documentation to ensure smooth transitions.

Qualifications

• 2+ years of patient facing and care coordination experience within healthcare, managed care, home health, and/or community health setting.
• Experience working in a telemedicine environment.
• Fully bilingual (English/Haitian Creole).

Responsibilities

• Reach out to patients while inpatient to introduce the TOC program and communicate post-discharge follow up from TOC clinicians.
• Schedule post-discharge follow ups (PCP/Specialty).
• Reach out to hospitals and provider offices to obtain clinical documentation.
• Submit Home Care and DME orders as well as vendor follow up.
• Monitor return fax queue and research provider profile for updated contact and fax information.
• Provide additional operational support for assigned team and manager as designated.
• Ensure patients are prepared for telehealth visits by assisting with technology prep.
• Collaborate internally in clinical rounds to discuss patients at risk for readmission.
• Coordinate lab testing and imaging, and tracking results to ensure review by ordering clinician.
