Job summary

Devoted Health is seeking a Care Coordinator to support TOC Clinical Operations in ensuring safe post-discharge planning and follow-up. You’ll engage with patients inpatient and post-discharge, coordinate with clinicians, and support documentation and scheduling to facilitate smooth transitions of care.

The role includes telehealth prep assistance, collaboration with internal teams, and handling lab/imaging coordination.

Qualifications

• 2+ years patient-facing and care coordination experience in healthcare, managed care, home health, or community health settings.
• Experience working in a telemedicine environment.

Responsibilities

• Reach out to patients inpatient to introduce TOC program and follow up.
• Schedule post-discharge follow ups (PCP/Specialty).
• Obtain clinical documentation from hospitals and provider offices.
• Submit Home Care and DME orders and follow up with vendors.
• Monitor return fax queue and update provider contact information.
• Assist with technology prep for telehealth visits.
• Collaborate in clinical rounds to discuss patients at risk for readmission.
• Coordinate lab testing and imaging and track results for clinician review.
