Devoted Health in the United States seeks a dedicated Bilingual Transitions of Care Coordinator to support TOC Clinical Operations. You will reach out to patients inpatient and coordinate post-discharge follow-ups, integrating care with PCPs and specialists.

Qualifications

• 2+ years of patient-facing and care coordination experience in healthcare, managed care, home health, or community health settings.
• Experience working in a telemedicine environment.
• Fully bilingual (English/Haitian Creole).
• MA certification (desired); expired certifications are OK.

Responsibilities

• Reach out to patients inpatient to introduce TOC program and communicate post-discharge follow up.
• Schedule post-discharge follow ups with PCP or specialty.
• Reach out to hospitals and provider offices to obtain clinical documentation.
• Submit Home Care and DME orders and vendor follow up.
• Monitor return fax queue and verify provider contact details.
• Assist with telehealth technology preparation for patients.
• Collaborate in clinical rounds to discuss patients at risk for readmission.
• Coordinate lab testing and imaging and track results for clinician review.
